Spero Hospice in Middlesex, NJ is seeking a compassionate and dedicated Hospice Nurse - LPN to join our team. • Deliver compassionate and comprehensive nursing care to terminally ill patients in their homes or in hospice facilities within Monroe Township, NJ.
• Work collaboratively with the interdisciplinary team to create and execute personalized care plans for each patient under your supervision.
• Ensure the effective management of pain and symptoms by administering medications and treatments as directed by the physician.
• Educate patients and their families on end-of-life care, symptom management, and strategies for comfort.
• Maintain precise and detailed documentation of patient assessments, care procedures, and outcomes in accordance with regulatory standards.

Qualifications:

• Minimum of 2 years of hospice care experience is required for this position in Monroe Township, NJ, US.
• A valid LPN license specific to the state of New Jersey is essential.
• Excellent communication skills are necessary to effectively interact with patients and their families.
• Must demonstrate the ability to deliver compassionate end-of-life care.
• Proficient in the administration of medications and monitoring patient conditions.
