Job Summary Wolfson Children's Hospital is hiring a Resource Navigator to join The Players Center team. This is a full-time, in-person position located on the downtown Jacksonville campus of Baptist Health.
Responsibilities include:
Provide assistance to patients and families by screening for social determinants of health
Identify opportunities to improve health
Connect families to appropriate resources
Follow-up to confirm the connections were made and beneficial
Assist families in enrolling children into Medicaid, Children's Health Insurance Programs), and other public benefits.
**This position requires attendance at evening and weekend events.
